A defect enters from a live room (the owner playing, on the box or on :8011), in the owner's words. It gets a root cause read against the code — never against the transcript alone — then a fix, then a pin: a smoketest check or an exam scenario that fails if it returns. A row is closed only when the pin exists; narrowed means measurably better with a known remainder; open means recorded, not yet built.
